Dedman has it! I was impressed by your first guess, where you got two out of three correct.
Just for your information:
3. Filmed my own series of silent superhero movies as a child
There are eight films all together, all done on Super-8 film, starring my friend as Incredible Boy, and me as various villains
4. Produced my own series of superhero "photo movies" (a photo story with accompanying audio track, the kind that beeps when you are supposed to turn the page) as an adult.
Two finished, a third in production, a fourth planned, all about "Maintenance Man"
5. Recorded my own "audio commentaries" for each of the season finales of Blake's 7 ("Orac", "Star One", "Terminal", and "Blake"
Never did this, although to my credit I did know the names of each of the season finales of Blake's 7!
6. Counted and tallied every character's word balloons in the 5 Years Later version of the Legion of Super-Heroes
If I remember properly, Jo Nah had the most "lines"
9. Left in the middle of Thanksgiving Dinner to take a bus to a Doctor Who convention
It was 1993, Visions convention in Chicago, to get there on time I had to leave on Thursday night from where we were having Thanksgiving in Virginia. I enjoyed the convention but I'm a bit embarrassed about this one.
10. Filled up the entire "Official Marvel Try Out Book" with my atrocious art
My art is atrocious, and I do own that book, but I never tried to draw in it.
11. Dressed up as the Time Trapper for Halloween
Never did this. Interesting idea, though. Imagine having to explain it to everyone. "No, I'm not death, I'm the Time Trapper..."

Hmm no guesses. OK. I visited Lash's Mom in Mississippi. I went to a spa in Phoenix. One summer the family vacation was to the Amish country. And I once got gas in Gary, Indiana while visiting Chicago.
So, the next poster should be able to get the correct answer. Don't wait for confirmation.
