After reading the Comet & Circe thread (in which we see Biron/Comet getting his super-powers as a result of Circe initially giving him the wrong potion), I was thinking of which other characters have super powers as a result of a mistake.
Bouncing Boy, by his own error of drinking the wrong liquid.
Maybe Sun Boy, since Regulus meant to kill him rather than empower him.

Perhaps Mekt, Garth, and Ayla too? The lightning beasts were trying to probably kill them or subdue them rather than empower them.
The Ranzzes most certainly weren't looking for lightning powers, just a way home.

Star Boy's Daxamite powers. Gained when he accidentally flew through the tail of a comet.

Streaky, who gained powers every time he got near a small Kryptonite stone that Supergirl had experimented on and then casually thrown into the woods!
The kryptonite later got wrapped up in a ball of yarn which Streaky would play with in the basement of Midvale Orphanage!

Ayla's light powers
while Nura intended them, she did so based on an erroneous prediction.

Wildfire?
It seems being clumsy around exotic tech is a good way to become a member of the gang.

Yes, Wildfire's powers preboot were born from an accident. Postboot, not so much.
Weren't Sunboy's powers from an accident? I don't rightly remember.

Less an accident and more an attempted homocide in the case of Sun Boy. So yes I guess he "accidently" lived

How about Clorophyl Kid? Does Vi's ability to grow count? Maybe even poor Zoe qualifies in a strange way. Look at all the times she went searching for a new power only to have Ra's throw her a curve ball.
