IB has a strong showing, being that Storm Boy was a 5 Year Gap Legionnaire and a member of the (very expanded) Superboy PRime LSV, but I was looking for the Substitute as well, and that means Myg has it!

And, by your logic, then Tellus, Quislet, Gates, XS, Blok and Dawnstar (at least) would have also qualified

Haven't thought about this much so haven't eliminated many possibilities, but...
They have all experienced bodily/form changes that have not been reversed?
Lu - lost one body
Salu - gained ability to grow
Drake - turned into anti-energy
Mysa - her albino form vs. her Naltorian form
Brin - Furball mutation which was never completely reversed
Jeckie - from snake to naga form
Although Monstress would also qualify...

Vi - became Leviathan upon gaining power to grow
Timber Wolf - Furball, need I say more
Projectra - expanded powers, became Sensor Girl
Mysa - from Hag to White Witch
Wildfire - used to be ERG-1. Postboot, was also Atom'x and Blast-Off
