Marella Tao and Circadia Senius? Circadia Senius is correct. He actually had a pretty bad-ass send-off in my opinion, calmly launching a means to replace Luna's missing mass blind because "I am an astrophysicist. If I don't know where the moon is supposed to be ... then we are all in k-k-considerable trouble." He then watched a chunk of Luna land a half mile away that would inevitably kill him. I'm unaware of Marella's fate, but don't recall her appearing after the first year of TMK in-story. It's possible the powersphere explosions that decimated Earth the same day may have done her in, but that was from the Dominators and not Luna. Circadia Senius and who else?

OK, IIRC, Cham implied that people who fell victim to the Chokeweed died (he was wrong, of course), but he knew of the Legionnaires who were Blight enforcers. That eliminates Ultra Boy and Karate Kid from my list, if my thinking is correct. So... Live Wire Invisible Kid Sensor Gates Kinetix

In "Legion: Secret Origins" Chris Batista produced some excellent work, including the covers; however, he drew a Legionnaire on one issue's cover that never appeared in story. Which hero did he try to add to the line-up?

I'm guessing it's either Star Boy or Shrinking Violet. Thom got swapped before publication for Ultra Boy, while Violet appeared on a cover but didn't actually join the team in-story. She had a cameo at the end of the series with Dirk and some others though via a projection screen of candidates.
